Welcome to Garden State Neurology and Neuro-Oncology! As a new patient, you can conveniently download essential forms from our website to streamline your onboarding process. These forms gather crucial medical information to help our expert neurologist provide personalized care. New Patient Forms:

Routine EEG:

  • Wash and clean your hair well in the morning or the night before the EEG.

  • Do not use any hair products – Do not put any spray, lotion or oil in your hair.

  • You may eat, drink and take prescribed medications as usual. Avoid caffeine.

  • Please do not sleep on the day of the EEG. The test is most useful if it contains drowsiness and sleep.

  • Please arrive at the EEG lab 15-20 minutes before your scheduled appointment.

Ambulatory EEG:

  • Wear a blouse or shirt that BUTTONS DOWN THE FRONT; this will make it easier for you to change clothes later if you wish.

  • Your head will be wrapped in gauze which can be covered with a scarf, loose fitting cap or hat if you would like to bring one with you.

 

Sleep deprived EEG:

  • Option 1: Do not go to sleep the night prior to EEG.

  • Option 2: Go to sleep around 12:00 to 1:00 and wake up at 4:00 to 5:00.

 

Do not use caffeine or any other stimulants to stay awake.

EEG Preparation

The patient does not need to do anything special to prepare for this test.
Please plan on being here for approximately one hour.
Please do not bring small children or babies with you, as they cannot be in the testing room during the procedure.

Be sure to inform the physician, however, if you are taking blood-thinning medication such as Coumadin, have hemophilia or a cardiac pacemaker.

 

Upper Extremities:

  • Wash you hands and arms well with soap.

  • Do no use skin lotions or creams on the day of the test.

  • Bring or wear a sleeve-less shirt or T-shirt.

 

Lower Extremities:

  • Wash you legs well with soap.

  • Do no use skin lotions or creams on the day of the test.

  • Wear loose shorts.

EMG Preparation

  • Wash and clean your hair well in the morning or the night before the EEG.

  • Do not use any hair products – Do not put any spray, lotion or oil in your hair.

  • You may eat, drink and take prescribed medications as usual.

 

Visual Evoked Response (VER)

  • Make sure and bring your glasses or contact lenses.

 

Brainstem Auditory Evoked Response (BAER)

  • If you wear a hearing aid make sure and bring it with you!

 

Somatosensory Evoked Response (SER)

  • This test can be ordered on arms, legs or all 4 limbs. It takes 45 minutes to test two limbs, and 90 minutes to test four limbs.

  • You will need to wear loose, comfortable clothing that will allow us access to your legs up to 5 inches above your knees, your neck, low back, and your hip. Two-piece outfits that have loose legs usually work well.
